New Patient Registration Times:
Monday 9:30 am - 1:30 pm
Tuesday 9:30 am-1:30 pm
Wednesday 2:00 pm-6:00 pm
To Register please complete the GMS1 form below and the Registration forms saved on the tab "Registering Online". Alternatively you can ask a Receptionist for a registration pack.
When you have completed the Registration forms please come to the Practice during the times above so that our Registration Administrator can register and book a new patient check for you there and then!
Remember when you are ready to Register yourself please bring a proof of address (a bill, hospital letter, etc) and a photo ID (passport, driving license) with your completed form to Reception.
The Practice area is defined by postcodes – B9, B25, B26, B33
Please see a Receptionist if you have any questions. The Registration Administrator will check you are within bounds when you hand in your completed forms.
Updating your Details
If you have changed your name, address or telephone number, please let our receptionists know by calling or writing to us. If you have moved outside the practice area you may need to find a doctor in your new area by visiting NHS choices.
